By signing this form, I, on behalf of myself, my heirs, executors and assigns, (or in the same manner on behalf of my participating child or ward) do release South Boston Catholic Academy, the USATF, all sponsors and their agents or employees from any and all responsibility for any injury or damage sustained arising in any way from participating in or attending the above event. I also represent that I am (or my child or ward is) in good health and adequately trained for the event entered. I grant full permission for the use of my child’s or ward’s name or likeness to report or promote this or subsequent events in any media including photographs, video or other recording devices. Entry fee’s are non-refundable. Every effort is made to honor shirt size selection, but exact shirts sizes cannot be guaranteed.
