Manhattan Mile

WAIVER FORM: In consideration of the acceptance of my entry, I the undersigned, my heirs, assigned executors and administrators hereby waive, release and discharge any and all rights of claims for personal injury, death or property damage which I may have or which may hereafter accrue as a result of my participation in, travel to or from the above subject event. This release is intended to discharge in advance the City of Manhattan Beach, the City and County of Los Angeles, Village Runner Racing, all promoters and sponsors or any individual or entities associated with the above subject event from and against any and all liability arising out of or connected in any way with my participation in said event, even though that liability may arise out of negligence or carelessness on the part of the persons, groups or entities mentioned above. I hereby attest and verify that I have full knowledge of all risks involved in this event and am physically fit to participate in said event. In the event of accident, injury or illness of the above named participant, consent is hereby given to any x-ray examination, anesthetic, medical or surgical diagnosis or treatment and hospital care which is deemed advisable by and is to be rendered under the general or special supervision of any physician and surgeon. I hereby agree to release to the race promoter full exclusive rights to record my performance in said race on film, videotape, or still photography for use without compensation.
