10th Annual Bridgewater Police 5k & 1mi Kids Fun Run

Bridgewater Police 5K and 1 Mile Kids Fun Run Release of Liability (Must be completed by all participants or by Parent/guardian if participant is less than 18 years of age.) In consideration of the opportunity to participate in the Bridgewater Police 5K and 1 Mile Kids Fun Run, an event which will take place on Saturday September 28, 2024, I, the undersigned, on behalf of myself, my administrators, executors, heirs, and assignees, or as supervisors for my minor children/ward, release, hold harmless and forever discharge and agree not to sue the Town of Bridgewater, the Bridgewater Police Department, its officers, employees, agents, or volunteers from any claims, demands, actions, and causes of actions of every name and nature resulting from or arising out of my participation in the Bridgewater Police 5K and 1 Mile Kids Fun Run whether or not caused by the ordinary negligence of any officer, employee, agent or volunteer. I understand, recognize and acknowledge that Town of Bridgewater and/or Bridgewater Police Department gives no assurances or warranties whatsoever as to the safety of participants in the Bridgewater Police 5K and 1 Mile Kids Fun Run, parts of the Bridgewater Police 5K and 1 Mile Kids Fun Run may be physically or emotionally demanding, notwithstanding precautions taken by the Town of Bridgewater and/or Bridgewater Police Department, the Bridgewater Police 5K and 1 Mile Kids Fun Run involves a risk of injury and/or death, and I am free to drop out of the 5K or 1 Mile race/walk at any time during my participation should I feel that my health is endangered. I am voluntarily participating in the Bridgewater Police 5K and 1 Mile Kids Fun Run with knowledge of the dangers involved. I grant permission to all of the foregoing to use any photographs, motion pictures, recordings or any other record of myself and/or my child/ward of this event for any legitimate purposes. I hereby agree to expressly assume and accept any and all risks associated with my participation in the Bridgewater Police 5K and 1 Mile Kids Fun Run. I carry valid and current health insurance for myself and/or my minor child/ward and have determined that this insurance is adequate to cover injuries or illnesses that I and/or my child/ward may sustain while participating in the Bridgewater Police 5K and 1 Mile Kids Fun Run. I will be solely responsible for payment in full of all costs of medical care I and/or my child/ward may receive. I authorize the Town of Bridgewater and/or the Bridgewater Police Department to obtain appropriate health care for me and/or my child/ward in the event that I, he/she, need it but am unable to obtain it for myself or am not available for my child/ward. I further agree to hold harmless and indemnify the Town of Bridgewater and/or Bridgewater Police Department and/or Bridgewater Fire Department for any and all actions taken by the Town of Bridgewater and/or Bridgewater Police Department and/or Bridgewater Fire Department to provide necessary emergency medical care to me during the Bridgewater Police 5K and 1 Mile Kids Fun Run. I also understand and agree that if I experience serious health problems, suffer an injury, or am otherwise in a situation that raises significant health and safety concerns, then the Town of Bridgewater and/or Bridgewater Police Department and/or Bridgewater Fire Department may contact any other person whose name I have provided as my “emergency contact.” I have read the foregoing and hereby agree to be bound by same.
